ABSTRACT
Blood transfusion are often taken place as part of the treatment of a great number of hereditary and acquired diseases, but imply potential dangers like immunological reactions and viral infections. Haemovigilance is resulting from the expression Pharmacovigilance, which encompasses activities and systems to collect information useful in supervising adverse drug reactions particular to medicinal products. Blood transfusion is very safe and effective when used appropriately. Between 1996 and 2003, 226 instances of ABO incompatible transfusion were reported to the Serious Hazards of Transfusion scheme. Sample errors Sample errors may be detectable by the laboratory, as is evident from Serious Hazards of Transfusion scheme data on ‘near-misses’ and by probable inspection. To reduce the risk of
these, all staff must receive training and competence assessment. Errors must be openly acknowledged to the patient / relatives, with a straightforward clarification and a warning of actions taken to prevent a recurrence. Errors are, by definition, unintentional, and disciplinary action is counterproductive except in the thankfully rare instances of blatant professional misconduct or criminal intent.
