UTILIZATION OF ANTIMICROBIAL AGENTS IN PAEDIATRIC INDOOR PATIENTS OF PNEUMONIA AT TERTIARY CARE HOSPITAL

ABSTRACT
Title: “Utilization of Antimicrobial Agents In Paediatric Indoor Patients Of Pneumonia At Tertiary Care Hospital.” Objective: To evaluate the utilization pattern of various antimicrobial drugs for treatment of pneumonia in paediatric indoor patients of KTCH, P.D.U. Govt. Medical college and suggest changes if any. Materials and Method: Patients admitted in the department of paediatric diagnosed as pneumonia, who met the study criteria were enrolled into the study and reviewed daily. A suitable case record form was designed to collect the all necessary and relevant information. A personal visit was made to all patients who were included in this study to collect further information. Result: Total 255 patients were included in study. Male
to female ratio was 1.6:1. Mean age of inpatients was 2.6 ± 2.9 years. During this period average number of drugs prescribed per inpatients was 4.91 ± 1.40. The average number of antimicrobials were prescribed per inpatients was 1.4. All patients received antimicrobials. Empirical antimicrobial therapy is started without testing for culture and sensitivity. Most commonly prescribed antibiotic is Amoxicillin + Clavulanic acid (64.87%) followed by amikacin (10.75%), ceftazidime (8.54%), vancomycin (8.54%), Amoxicillin (4.11%), azithromycin (3.16%). Antimicrobials prescribed were included in Essential medicine list of WHO. Three antimicrobials were prescribed from National Essential Medicine List. Antimicrobials were prescribed by parenteral route in 90.9% of patient and by oral route in 9.1% of patients. Only two ADRs were reported during the study. 215 patients were discharged, 34 had taken discharge against medical advice (DAMA), while 6 died during course of therapy. Conclusion: Average number of antimicrobials prescribed per patient was lower than other studies. Majority received a single antimicrobial agent. Most commonly prescribed antimicrobial agents were Penicillins, Aminoglycoside followed by Cephalosporins. Prescription of antimicrobials by generic name was higher. Parenteral route was preferred for antimicrobial agent. Rate of addition or change of antimicrobials was lower than other study.
