A CASE REPORT ON ANTINEUTROPHILIC CYTOPLASMIC ANTIBODY (ANCA)- ASSOCIATED VASCULITIS(AAV): FROM RENAL INVOLVEMENT TO END-STAGE RENAL DISEASE

ABSTRACT
Antineutrophilic Cytoplasmic Antibody (ANCA) -associated vasculitis (AAV) is an enormous challenge, particularly in renal involvement, as these diseases precipitate chronic kidney disease (CKD), in extreme cases leading to End Stage Renal Disease (ESRD). This case report presents a 54-year-old female with a history of past AAV, presenting with marked anasarca and oliguria, the case suggesting progressive deterioration of renal function. Irregularity in adherence to immunosuppressive therapy had worsened the condition, so much so that renal parameters were deteriorating at a speedy rate. Laboratory reports had shown an elevation in serum creatinine levels. The case establishes the fundamental role of early diagnosis and regular treatment to avoid irreversible kidney damage. It requires a multidisciplinary approach in patients by incorporating nephrology and rheumatology. The management with conscious patient education would be optimal with frequent monitoring for better adherence to treatment protocols. Thus, proactive management can improve the quality of life of those patients afflicted with AAV through better long-term outcomes.
