Abstract

A COMPARATIVE CLINICAL STUDY ON THE EFFICACY OF MADHUCHISTADI ROPANA GHRITA AND CHANDANADI YAMAKA IN DURDAGDHA VRANA

ABSTRACT

Introduction- Burn injury is one of the top causes of non predictable and disabilities worldwide. Most of the burn are accidental, need early care without which can threaten the life. Burn trauma represents a type of injury that can be caused by heat, freezing, electricity, chemicals, radiation or friction. Concept of burn and its management is found in Ayurveda from the era of Sushrut Samhita to the modern era of surgery. In Sushrut Samhita description of etiopathology, clinical features, diagnosis and management of burn wound is found where it is described as first degree (plusta), second degree (durdagdha), third degree (samyak dagdha) and fourth degree (atidagdha) with different principles of management. There are different types of Ayurvedic ointments described in classics for different degrees of burn injuries which are relevant in present era of surgical practice. Aim- Tocompare the efficacy of Madhuchistadi ropana ghrita and Chandanadi Yamak in durdagdha vrana as dressing materials. Material & Method- Initially 64 numbers of patients were selected as per inclusion and exclusion criteria for this study in 2 group (Group A and Group B). 2 number of patient of each group left from the study due to personal cause. Daily dressing of the wound was done in the morning following all aseptic measures. Two groups of 30 patients each were made and after proper debridement & irrigation, Madhuchistadi Ghrita was smeared on the wound surface gently in Group A and Chandanadi Yamaka in the Group B followed by covering the wound with sterile bandage. Data were recorded in a Standard proforma which include all the subjective and objective parameters. Result- Both the preparations showed interesting results in terms of tissue replacement and epithelization. However Madhuchistadi ropana ghrita has been found more efficacious in the management of durdagdha in terms of wound bed preparation, epithelialization & prevention of contracture and other complication etc. Discussion & Conclusion- Ghrita has the property of Ropana and Samskarasya gunanubartanam. Madhuchistadi ropana ghrita, after smearing over the burn wound surface, creates an environment for healing as well as preventing evaporation loss. The drug is very effective in terms of duration of healing, promote different grow factors and prevention of stricture and keloid.
