ABSTRACT
Introduction: Bertolotti's syndrome (BS), also known as lumbosacral transitional vertebrae, presents a distinctive anatomical variant characterized by an anomalous enlargement of the transverse process of the most caudal lumbar vertebrae, potentially fusing or articulating with the sacrum or ilium bone. Case Study: A19-year-old female with six-month LBP history, unresponsive to conservative measures, who underwent L5-S1 Transforaminal Lumbar Interbody Fusion (TLIF) with satisfactory outcomes, with no other significant medical conditions reported. postoperative management comprising intravenous administration of antibiotics, Analgesics, PPI with regular dressings. The patient's intraoperative and postoperative courses were uneventful, with satisfactory alignment confirmed on postoperative X-ray. She was discharged in a hemodynamically stable. Discussion and conclusion: This underscores surgical intervention's significance in refractory LBP cases. Comprehensive preoperative assessment is vital, emphasizing surgery's role as a viable treatment option in appropriate BS cases.
