Abstract

A PROSPECTIVE STUDY ON INCIDENCE OF DIURETICS INDUCED ELECTROLYTE IMBALANCES IN CARDIAC PATIENTS AT A TERTIARY CARE HOSPITAL

ABSTRACT

Introduction: Electrolytes are the smallest of chemicals that are important for the cells in the body to function and allow the body to work. It produces an electrically conducting solution when dissolved in water. There are three hormones that play key roles in regulating fluid and electrolyte balance. Antidiuretic hormone (ADH) Aldosterone secreted from the adrenal cortex Atrial natriuretic peptide (ANP) produced by the heart. Cardiovascular disease (CVD) is a general term for conditions affecting the heart or blood vessels. Method: This study was carried out in Multispecialty Hospital, This study was conducted over a period of six months A total of 100-120 adult and geriatric patients were included in the study & data collected from patients Reults: The subjects with ideal BMI are (28%), of overweight are(35%) and obese are 37% irrespective of their age & gende. Past history of Hypertension (HTN) and Diabetes Mellitus (DM), Coronary Artery Diseases (CAD) are observed. The subjects are mostly treated with diuretics. Out of 119(100%) patients treated with Loop Diuretics and its combinations, 67 (56.3%) patients are treated with Torsemide combinations and 52(43.6%) are treated with Frusemide combinations. Among all patients, 39(32.7%) are treated by Torsemide + Spironolactone, Conclusion: Hyponatraemia and hypokalemia are the most frequent electrolyte disorders in clinical practice. Electrolyte disturbance is common after administration of Loop and Thiazide diuretics. hyponatraemia electrolyte disorder is often thought to be rapid in onset, unpredictable and idiosyncratic. Hypokalemia induced by high-dose diuretics has been associated with ventricular arrhythmias and cardiac arrest. Aim: To study the incidence of electrolyte imbalances associated with diuretics being taken on admission in cardiac patients at a tertiary care Hospital. Objective: The objectives of the research work undertaken as follows:1. To observe the diuretic associated electrolyte imbalances in cardiac patients.2. To evaluate any other reported complications due to electrolyte imbalance.3. To perform a statistical analysis among selected cardiac subjects.4. To improve overall patient quality of life.
