MANAGEMENT OF GUTTATE PSORIASIS. (KITIBHA KUSTHA) - A SINGLE CASE STUDY

ABSTRACT
Background of study: Guttate psoriasis lesion mostly present over upper trunk, proximal extremities. In present scenario, skin diseases difficult to treat because they already taken allopathic medicines, our Shamana Chikitsa is not sufficient to treat so, there is need to give such patient. Kshetrikaran i.e Shodhana Chikitsa, hence present study just little try to give line of management. The negative influence of psoriatic disease on patient's physical and mental wellbeing in combination with overlapping pathophysiology, it increases the risk for clinically significant psychiatric Conditions. AIM: To access the management in Guttate psoriasis by using Shodhana, Shamana, Bahirparimarjana Chikitsa. Objectives: 1) To study causative factor
of Kitibha Kustha. 2) To analyse the therapeutic effect of Shodhana, Shamana and Bahirparimarjana Chikitsa. 3) To restrict recurrence of Kitibha Kustha. Methodology: A 35yrs male patient approached to skin OPD, GAC-Nagpur having Complaint of multiple small papules (0.5-2cm in diameter) over upper trunk, upper extremities, back region with profuse itching, epidermal proliferation, Red coloured lesions since 2 month. -According to Acharya Charaka, Kitibha having Lakshanas like Shyava, Kina, Khara Sparsha, Parusha, Ruksha Pidika and Kandu which are accessed by PASI Score. -Here stress factor is one of the main cause as per it’s clinical symptoms. We treated it as Kitibha and according to their basic principles. such as Shodhana (Virechana), Shamana (Shirodhara and Internal medications) and Bahirparimarjana (Sidhartaka Snana). Result: Patient got 90% result in chief and associated complaints within 2 months. He get improvement of quality of his life. Conclusions: These case study concluded that patient having Pitta-Varaj Prakruti with typical vitiated Pitta-Vataja presentation. Kitibha Kustha can be successfully treated with Shodhana, Shamana and Bahirparimarjana Chikitsa. It is recommended to conduct the research study on more number of patients to prove basic therapeutic principles of Kustha.
