Abstract

EFFICACY OF JALAUKAVACHARANA IN THE MANAGEMENT OF ARSHA W.S.R. TO THROMBOSED PILES

ABSTRACT

Arshas are one of the most common diseases occurring in ano-rectal region. Though the disease is within the limits of management, it has its own complications like sever hemorrhage, strangulation, thrombosis, etc, thrombosed piles is one type where blood clot forms inside a hemorrhoidal vein, obstructing blood flow and causing a painfull swelling of the anal tissues, Acharya Susruta mentioned four folds of treatment modalities i.e ausadh, Shastra, Kshara & Agni for Arshas Vyadhi. Jalokavacharana is one such treatment indicated in removal of obstructed blood from local site. Hence this present study is aimed to evaluate the efficacy of Jalokavachara (leech therapy) in the management of Thrombosed piles. Methods- 30 patients were selected on the basis of diagnostic criteria of thrombosed piles. Jalokavacharana was done by following good surgical practices after proper proctoscopic evaluation. The signs and symptoms like burning associated with defecation, bleeding per rectum, constipation, degree of pile mass, colour of pile mass and tonicity of anal sphincter were assessed before and after the treatment. Results- tonicity of anal sphincter were completely reduced in all patients with in the period of 30 days. No recurrence was found in all patients after 30 days of follow up. Conclusion: One of the main clinical symptoms of Arshas is bleeding per rectum. The main advantage of this study was the complete relief in the symptom of bleeding per rectum within 2 weeks of Jalouakavacharana. Overall all signs and symptoms were completely relieved in all patients with after a period of 30 days after the treatment. Hence this can be considered as as one of the best modality for the treatment of thrombosed piles.
