CHRONIC DISORDER MANAGEMENT BY PHARMACISTS IN GERIATRIC PATIENTS

ABSTRACT
Chronic disorders are the major health issues that cause death worldwide. The management of the chronic condition is a major concern in today’s scenario. The pharmacist play a lead role in healthcare professional group in the world after physicians and nurses. Pharmacists are the only medical professionals that are mainly for providing medication counseling to patients, despite their extensive training, and as a result, care of public health. Pharmacists might boost integrated primary care delivery across the health system by lowering drug-related adverse events and encouraging greater prescription
adherence, which in turn may aid in lowering needless provider visits, hospitalizations, cost-effectiveness and readmissions.
