PAINFUL LOWER LIMBS AND ERYTHEMA NODOSUM IN A PATIENT WITH DIABETES MELLITUS UNMASKING SARCOIDOSIS: CASE REPORT

ABSTRACT
Sarcoidosis is a granulomatous inflammatory disease which may involve multiple systems of body collectively or in isolation. It is known to cause hilar adenopathy, erythema nodosum, peripheral neuropathy and much more. Here we present a case of long standing diabetes mellitus on medications who presented with severe pain in both lower limbs of few days, which did not respond to multiple pain alleviating drugs as paracetamol, tramadol, flupirtine, etizolam-escitalopram and pregabalin. Within few days of presenting to hospital he developed erythema nodosum and subsequently sarcoidosis was
diagnosed which beautifully responded to naproxen and low dose steroid. Hence erythema nodosum can be a potentially diagnostic window for diagnosis and should be always kept in mind. Also other causes of bilateral lower limb pain should be always kept in mind and investigated accordingly such as sarcoidosis warranting clinical examination of lower limbs.
