CRITICAL REVIEW ON NEONATAL UMBILICAL CORD CARE

ABSTRACT
Introduction: Neonatal mortality and morbidity due to neonatal sepsis is a global burning health issue. Umbilical cord stump is a great source of infection for sepsis in newborn. According to modern science, the umbilical cord forms by the fifth week of fetal development, replacing the yolk sac as the source of nutrients for the foetus after birth, the umbilical cord serves as the lifeline to the foetus, as it supplies all the nutrition. A number of abnormalities can affect the umbilical cord, which can pose threat to the wellbeing of baby, and sometimes mother as well. Improper cord care methods are the chief cause of most of the umbilical diseases in neonatal period. So, care of umbilicus and management of umbilicus disorders are one of the important areas of interest in paediatric practice. Till date health professional are
struggling with the standard cord protocol which prevent the colonization of bacteria causing sepsis, protect the bacterial colonies useful for healing the wound after cord fall, help in early separation of cord and to prevent the future complication like omphalocoele. Aims and Objectives: (1) To review the Ayurvedic and modern concepts of neonatal umbilical cord care and management of related disorders. (2) To facilitate a safer, effective and rationalized umbilical care in today‟s scientific era. Materials and Methods: Ayurveda and Modern literature, Scientific Journals, Published research and Articles. Discussion and Conclusion: Umbilical cord care starts with delivery of baby itself. Cord cutting instruments should be clean and hygienic (as per Ayurveda classics – made up from specific metal), baby‟ environment should be dry and hygienic and minimum handling of cord stump should be maintained (as per Ayurveda classics, Grivayam Avsanjayet). And if at all, any Nabhipaka take places, medicated oil prepared with Haridra, Lodhra, Madhuyasthi, Priyangu and Devdaru can cure the Nabhipaka (umbilical sepsis). As per local tissue condition dusting of same drugs can also be used effectively. Herbal drugs like these, resists completely the growth of infection from gram + ve Diplococci, gram + ve Bacilli, Klebsiella, pneumoniae etc. Herbal drugs are very effective due to its Vranaropak, Vranashodhak, Shothaghna, Shulaghna, Raktashodhak and Raktastambhak property.
