ROLE OF VARIOUS TYPES OF KSHARASUTRA IN THE MANAGEMENT OF FISTULA IN ANO

ABSTRACT
In Sushrut Sanhita many surgical diseases were mentioned. Bhagandara (Fistula in ano) is a common anorectal disease occurring in all age group. It is a track that connects deeply the anal canal or rectum to the skin around the anus and lined by granulation tissue.[1] The incidence of fistula in ano developing from anal abscess ranges from 26% - 38%. Prevalence of fistula in ano is 8.6 cases per 1 lac population. In men 12.3 cases per 1 lac population. In female 5.6 cases per 1 lac population. Male to female ratio is 1.8: 1. In modern science fistulotomy and fistulectomy are the operative procedures mentioned
for management of fistula which has high rate of recurrence. Sushrut has mentioned Chedan karma (surgical excision) for the management of Bhagandara.[2] He has also mentioned the Ksharasutra in Nadi Vrana (Sinus), Bhagandara, arbuda (Benign tumour) chikitsa.[3] In ancient days due to unavailability of anaesthetist drugs, fear for surgery Ksharasutra treatment flourished over Chedan karma. This has been proven that Ksharasutra therapy has relatively lower rates of recurrence. Now a days various types of Ksharasutra like Apamarg Ksharasutra, Ghritakumari (Aloe Vera) Ksharasutra, Udumbara Ksharasutra, Yavakshara coated Sutra, Erandkarkati (Papaya) Sutra and Guggulu based Ksharasutra also used in the treatment of Bhagandara. This article highlights role of various types of Ksharasutra in management of Bhagandara (Fistula in ano).
