PHARMACIST INTERVENTION FOR A FRAIL ELDERLY POPULATION IN PRIMARY CARE

ABSTRACT
The primary health care system of an area or a country is the most traditional and significant. Developing countries are too much rely on their primary health care system. Pharmacists play an integral part in this system and the aging population is the common patient. This article aims to identify the various role of pharmacists in primary health care and also the common problem while doing this. There is a discussion about the elder population and their health. How to maintain a healthy life and what will do when someone is suffering from comorbidity. This article will give a focus on pharmacists' role and primary care of the frail elderly population. Hope that in the future there will be more discussion around this topic.
