Abstract

COMPARATIVE CLINICAL STUDY OF NAVAMA MAASA PARICHARYA WITH MAHANARAYANA TAILA AND KSHEERABALA TAILA ON PRASAVA LAKSHANAS

ABSTRACT

Introduction- “Labor is defined as a series of events that takes place in the Genital organs in an effort to expel the viable products of conception out of the womb through the vagina into the outer world”. Changes in the uterine cervix tend to precede uterine contractions. Success of uterine contractions depend on normalcy of 3 P’s- Power Passenger and Passage. Any abnormalities in these factors leads to operative deliveries. Classical references in Ayurveda explains that labor process is governed by normalcy of Apana vata with the property of Garbha nishkramana. Vyana vata with its the properties of Gati, Akunchana, Prasarana help in initiating and stimulating the uterine contractions. Incidence and Prevelance- In India the rate of cesarean section delivery has increased from 3% to 10% between 1992-93 and 2005-06.The increase in the rate of cesarean section is a global phenomenon and hasn’t contributed to an improved pregnancy outcome, maternal mortality and morbidity. Incidence of operative interference in labor is increasing due to obstructed labor, malpresentation, fetal distress, arrest of dilatation and many other obstetric indications. Methodology- As a part of Navama masa Garbhini Paricharya, Acharyas have explained Anuvasana Basti, and yoni pichu with Madhuroushadha Siddha Taila for sukhaprasava without any complications to the mother and child. Minimum of 30 primigravida women with 37 weeks of gestation who had fulfilled the inclusion criteria were selected from outpatient department and inpatient department of Prasuti tantra Evam streeroga department of SDM college of Ayurveda and Hospital, Hassan Karnataka were included in the study. Group 1 – 15 Subjects were given Navama Maasa Paricharya with Mahanarayana Taila and Group 2- 15subjects were given Navama Maasa Paricharya with Ksheerabala Taila. Navama Maasa paricharya consisting of Matra Basti and Yoni pichu. It was started after 37th completed weeks and done till the onset of labor. Results: Navama Maasa paricharya was found effective in inducing the Premonitory stages of 1st stage of labor and cervical ripening Stage. Navama Maasa paricharya proved effective in improving the Bishop’s score especially in Cervical dilatation and Cervical consistency and helped in changes of cervix. But it was not found effective in Progressing of all Prasava lakshanas because the description given by acharyas include all the three stages of labor and not only 1st stage stage hence was found Statistically and clinically non-significant. Discussion- Navama Maasa Paricharya consists of Matra basti and Yoni pichu. These two modalities have the properties of vata anulomana, vata shamana, garbhashaya marga snehana. Mahanarayana taila contains 32 madhura rasa(anurasa) pradhanadravyas, which has properties such as Drava, Sara, Snigdha, and is Balakara. Ksheerabala taila is Guru, Snighda, Madhura rasa pradhana Taila having Rasayana, Jeevaniya, Balya and Brumhana properties. These tailas have added benefits in clinical practice, helping in initiation of labor. The present study is the clinical study of comparative clinical study of Navama Maasa paricharya with Mahanarayana taila and Ksheerabala taila on prasava Lakshanas.
