ABSTRACT
A 40 year old female reported to the Department of Conservative Dentistry and Endodontics in Babu Banarasi Das College of Dental Sciences, Lucknow with a chief complaint of pain in her upper left back tooth region. On taking history, it was noted that the patient had undergone root canal treatment in relation to upper left back region of tooth two months back(May 2019). Later on, the patient noticed pain in the same tooth after 2 months (July 2019). On taking radiograph, the IOPAR revealed incomplete RCT with fractured instrument of 6-7 mm apically, of which 1-2 mm was extruded outside the apex. Intentional replantation was planned for this case as the conventional endodontic re-treatment or surgical endodontics could not be performed. Even though the long term prognosis of this case is uncertain, this treatment plan has proven to be quite effective and is also advantageous to the patient as was seen after a follow up of 1 month. The tooth is functional, firm and free of any symptoms, thus relieving the patient. Therefore, it can be a reliable and predictable procedure if proper case selection is done and all the protocols are followed.
