DRUG-RELATED PROBLEMS AMONG CKD PATIENTS

ABSTRACT
Chronic kidney disease occurs with multiple co-morbid conditions and is a very common problem which leads to most drug related problems. Proper identification and management of these problems improves the quality of life. Patients on stage 5 CKD requires more than 10 medications which leads to polypharmacy. Challenges are involved in actually taking all of these problems into consideration. The role of clinical pharmacist is much needed to fill in with the existing gap in health care settings.
