Elderly patients are prone to adverse drug outcomes due to age-related physiological changes and the complexity of their medication regimens. Integrating clinical pharmacists in multi-professional healthcare teams has been found to be cost-effective for improving the appropriateness of prescribing and optimizing medication usage.
