ABSTRACT
Increased progesterone levels during pregnancy may cause decidualization of endometriomas mimicking malignancies on radiology and causing management dilemmas. This report describes a unique sonographic and MRI appearance of ovarian endometrioma with decidualization, which mimicked ovarian cancer. In women with ovarian endometrioma during pregnancy, uncommon entities such as decidual change of endometriosis should be considered. The decision for surgery depends on the level of suspicion of malignancy, the gestational age at presentation, and the risk to maternal/fetal morbidity including preterm labor and delivery.
Keywords: endometrioma, MRI, decidualization, pregnancy ovarian cancer.
