ABSTRACT
Arsha is explained as one among the Ashta Mahagada[1] (Eight grave diseases) that causes pain like an enemy and creates obstruction in Guda Marga. It is one of the anorectal disorder mentioned in Ayurveda with four-fold treatment modality as per severity, and it is correlated to piles as per modern science. Acharya Sushruta has indicated Kshar -Pratisaran as a Para-Surgical procedure in Arsha having symptoms like Mrudu (soft), Prasruta (spreaded), Avagadh (deep rooted), Uchhrita[2] (bulging out),which can be correlated with second degree internal haemorrhoids. Arsha (Internal Haemorrhoids) is said to be managed very well by local application of Kshar which creates chemical burn to the pile mass & produces shrinking effect on pile masses. Later it sheds off after necrosis. Considering all these factors, the present case study was undertaken to assess the role of Apamarga
Pratisarniya Kshara Lepa on Arsha (second degree internal haemorrhoids). One diagnosed case of Arsha (Second degree Internal Haemorrhoids) was taken from O.P.D. of P.G. Department of Shalya Tantra, Rishikul Campus, U. A. U Haridwar and was treated with local application of Apamarga pratisarniya Kshara-lepa. After the local application, patient was assessed for relief in sign and symptoms at weekly interval for two month as a follow up period. There was considerable relief in bleeding, pain and prolapse of pile masses. Hence Apamarga Pratisaraneeya Kshara lepa can be successfully used in Arsha with less post-operative complications.
