A PROSPECTIVE STUDY ON DETECTION, EVALUATION AND REPORTING OF MEDICATION ERRORS IN INTENSIVE CARE UNITS AT TERITIARY CARE TEACHING HOSPITAL

ABSTRACT
This study was conducted at a tertiary care teaching hospital with an aim to figure out and evaluate intensity of various types of medication errors at Intensive care units. Study helps to find reason and cause of different medication errors and helps to evaluate its severity and improve patients medication adherence as well as quality of life, it also reduces patient hospital burden. It was a prospective and observational study. In my study 115 patients were enrolled out of 115 cases 289 medication errors are evaluated and reported, Majority of errors are reported at ICCU 40% followed by neurology, Errors are reported more in male patients than female. About 224 errors occurred due to nursing staffs followed by physicians. Most common error occurred was administration errors followed by transcription error. Study ends with the conclusion that clinical pharmacist is necessary health care provider in detection and reporting of medication error and thus can prevent medication errors and can improve the quality of life of patients.
