DE NOVO HIGH GRADE NEUROENDOCRINE CARCINOMA OF PROSTATE

ABSTRACT
Prostate carcinomas is the second most common cause of death in the world. Adenocarcinoma of prostate is the most common carcinoma in prostate accounts to 95% of prostatic carcinomas. Neuroendocrine carcinoma accounts to 0.5% to 2% of all prostate carcinomas. High grade neuroendocrine carcinoma of prostate is very rare. High grade NE carcinoma has been reported in a very few case series with only 15 cases reported till date. Herein, we present a case of a high grade NE carcinoma diagnosed in a 70 -year-old man who presented with urine retention and gross haematuria. An enlarged prostate with bilateral multiple nodules on both lobes was detected in digital rectal examination for which transrectal ultrasonic biopsy of prostate was performed. Histopathology of the TRUS specimen showed a high-grade large cell NE carcinoma which was confirmed through immunohistochemistry. Computed tomography scans of the abdomen and pelvis revealed neoplastic lesion of prostate with extra prostatic involvement and metastatic pelvic lymphadenopathy. He subsequently underwent 6 cycles of neoadjuvant chemotherapy followed by radical retropubic prostatectomy and pelvic lymphadenectomy.
