Abstract

HORMONAL REASSESMENT IN HIRSUTE WOMEN

ABSTRACT

Background: Hirsutism is not merely aesthetic problem, it can be linked to several metabolic and endocrine abnormalities especially if a patient presents with moderate-severe Hirsutism even when it had a slow progression or family history can be obtained. Hirsutism is a distressing and relatively common problem, affecting 5 to 10% of women in the reproductive age group. It is usually a sign of androgen excess, the commonest cause being the polycystic ovarian syndrome (PCOS). Diagnosis depends mainly on assessment of the clinical symptoms and laboratory values of various hormones. Anti-androgens combined with various cosmetic therapies form rm the mainstay of treatment. Patients and methods: The sample of this study consist of 100 women 50 with Hirsutism were included in this study, their ages ranged from 14-45 years (SD. 27.64 + 7.266). Mean duration of disease was 7.52 + 6.217 years; while mean age of onset 20.36 + 4.758 years. All patients had slow progression of Hirsutism over years. Seborrhea, androgenetic alopecia, acne and acanthosis nigricans were recorded in patients with decreasing frequencies, while hypertrichosis was found in 72% of patients especially in those with high score Hirsutism, and the other 50 women were normal regardes as control group. Results: of this study indicate that in general there was nearly no significant relations between hormonal assay (androgen) and Hirsutism in patient group and normal group, in fact we notes from this study that the 3 hormones (testosterone, progestrone, and prolactine) was slightly higher in normal group than in patient group within normal values in comparism to other studies. With taking in the consideration that there was significant differences in relation to different clinical symptoms like irregular cycle or infertility. etc. Conclusion: Hirsutism can give a clue to underlying endocrine disorder especially if patients have high score of Hirsutism, thus it should not be ignored as it simply represents a cosmetic, racial or familial problem, in this study and previous study done by Sharquis et al (1986) there were normal androgenic hormons in high percentage of Iraqi women, there for we should lock for other factors that play a role in increase the chance of Hirsutism in population, Iraqi women were regarded hirsute in comparison to other athnic women.
