ROLE OF TILAKALKADI LEPA (PASTE) AFTER APPLICATION OF PRATISARANIYA KSHARA AFTER FISTULOTOMY IN LOW ANAL FISTULA IN ANO (BHAGANDARA) - A CASE STUDY

ABSTRACT
The Bhagandara (fistula in ano) is an abnormal communication between the rectum and anal canal to the perineal skin. Clinically present as single or multiple openings in the perianal area (Guda Pradesh) with various types of discharge and mild to severe pain. Acharya Sushruta described the Chhedan karma, similar to the fistulotomy, followed by the application of Kshara for the treatment of the Bhagandara. This procedure causes a wound, which requires regular dressing and care because of its location at the anal region, which is continuous remains in contact with fecal material so more prone to infection and required proper dressing. There are several
antiseptic solutions and topical agents available to create favorable conditions for wound healing but there are some drawbacks of these agents such as damage to the healthy granular tissue, local skin irritation, etc. Sushruta described the Tilkalkadi lepa to enhance wound healing. Thus, a study carried out to know the efficacy of this herbal compound in fistulotomy wound of a single patient with low anal fistula. It can be concluded on the basis of this study that Tilkalkadi lepa having good wound healing properties as well as reducing the pain on local application.
