MANAGEMENT OF BHAGANDARA BY NIMBA KSHAR-SUTRA

ABSTRACT
In Ayurved, Acharya Sushruta described Bhagandara (Fistula-in-ano) under eight major diseases (Ashtamahagada). Bhagandara has been described as five types Vatika bhagandara (Shataponaka), Paittika bhagandara (Ushtragreeva), Kaphaja bhagandara (Parisravi), Sannipataja bhagandara (Sambhukavarta) and Agantuja bhagandara (Unmargi). Acharya Sushruta has mentioned that all types of Bhagandara are difficult to treat. In modern surgery also, it is known for its callus nature to cure with high recurrence rate. In many of the cases, recurrence is seen after being treated by modern surgical conventional methods like fistulectomy or fistulotomy. To answer such a challenging problem in a comprehensive manner, Shalya Tantra in
Ayurveda stands with the positive effect of Kshar Sutra therapy. Kshara is the best among all surgical and para-surgical measures; Acharya Sushruta has mentioned many Kshara preparations in the text Sushruta Samhita. In the present research work Nimba Kshar Sutra application was done for the management of Bhagandara. 20 diagnosed cases of Bhagandara were selected from OPD and IPD of P.G. Department of Shalya Tantra, Rishikul Campus, Uttarakhand Ayurved University, Haridwar (U.K.) India. The results showed significant relief in subjective and objective parameters along with complete cure. Moreover, this therapy was well accepted by all patients and did not cause any interruption in their daily routine work during period of management. No complications were observed in this clinical study with follow up period.
