ABSTRACT
Torsion of the adnexal is a rare pathology, secondary to the total or partial rotation of the adnexal around its vascular axis. It poses a diagnostic and management problem despite radiological progress. The torsion manifests as severe, severe pelvic pain, sometimes associated with nausea or vomiting. The diagnosis is evoked by clinical examination and by ultrasound with Doppler, finding an ovarian cyst with interruption of the vascularization of the adnexal. It is a surgical emergency because a delay in treatment risks causing necrosis with loss of the adnexal. The operation is done by laparoscopy and consists of untwisting the ovary or performing an annexectomy in the event of late diagnosis with necrosis. This explains the constantly high rate of
adnexal losses. The urgency becomes more extreme when there is a bilateral torsion of the two ovaries because it involves the prognosis of fertility.
Keywords: Adnexal; bilateral twist; ovarian cyst, fertility.
