MANAGEMENT OF BHAGANDARA (FISTULA IN ANO) BY KSHARA SUTRA – A CASE STUDY

ABSTRACT
A fistula-in-Ano is an abnormal connection between the peri-anal skin and the anal canal. It is a hollow tract lined by granulation tissue and epithelial cells that are similar to the covering of the skin and anal canal. It starts as a small boils with abscess that bursts through the inner and outer opening and with incomplete healing the tract forms. Sometimes there may be one inner opening and two or more outer openings. Any opening in perianal area with chronic pus discharge indicates fistulous tract. Prolonged sitting, unhygienic condition, obesity, repeated irritation due to hair may increase the risk of occurrence. In Ayurved it is correlated with Bhagandara, among them, Bhagandara is considered under the Ashtamahagada by Acharyas of
Ayurved. Acharya Sushruta mentioned five types of Bhagandara. He had explained Shastra karma along with Kshara karma and Bhesaja Chikitsa for treatment. Acharya Charaka mentions the line of treatment for Bhagandara. Ksharasutra therapy is a radical cure in the treatment of Bhagandara without complications and recurrence. Here a case of fistula-in-ano in a 40 year male patient was examined and treated with Kshara Sutra, considering it as an ideal procedure in treatment of Bhagandara.
