MANAGEMENT OF SUTIKA UNMADA THROUGH AN INDIGENOUS TREATMENT MODALITY; A CASE STUDY

ABSTRACT
Sutika unmada or postnatal psychosis is considered as a psychiatric emergency due to both of suicidal and infanticide attempts. Having high prevalence rate among puerperal mothers, applying an effective approach is essential. In Ayurveda, among 74 varieties of sutika roga, the name is only mentioned, other than no any literature evidence available with both of Vruddhathrayi or Laghuthrayi. The current case study was focused on management of sutika unmada through an indigenous treatment modality which is inherited to Neelammahara psychiatry tradition of Sri Lanka. Effectiveness of Neelammahara Psychiatric treatment protocols in the management of sutika unmada and the improvement in quality of life of sutika unmada case through
Neelammahara psychiatric approaches were specially observed. As subjective parameters, shila, achara, cheshta, buddhi, manas, smruthi and sajnajnana of the case was observed. In objective parameters, hemoglobin count, thyroid profile, serum lithium concentration and body mass index were studied. Additionally, Edinburgh Postnatal Depression scale was used to observe quality of life of the case. Memory enhancement, improving quality of sleep, encouraging the case, regaining the mental stamina through effective insight and perception was idealized through the study. The study revealed that Neelammahara has positive approaches towards sutika unmada case through their indigenous treatment modalities. Application of yuktivyapashraya chikitsa along with daivavyapashraya and satvavajaya chikitsa was supported in promoting quality of life of the case. Psychoeducation and psychosocial support given through satvavajaya chikitsa was beneficial in preventing relapses.
