Abstract

PREVALENCE OF MULTI DRUG RESISTANCE, EXTENSIVELY DRUG RESISTANT AND PAN DRUG RESISTANT PROTEUS SPECIES WITH EMPHASIS ON ESBL PRODUCTION

ABSTRACT

Background: Proteus species are among the commonly implicated pathogen in hospital as well as community acquired infections. Emergence of infections caused by ESBL, MDR, XDR and PDR Proteus is alarming which creates serious health problem resulting in an enormous burden in health care setup and cost. Aim: This study was aimed to determine the prevalence of multi drug resistant, extensively drug resistant and pan drug resistant Proteus species. To find out the rate of ESBL production in the isolates obtained. Materials and Methods: 4356 different clinical samples received in the microbiology laboratory from patients admitted in MMIMSR, Mullana, Ambala, over a period of 6 months. Isolation, identification and antimicrobial susceptibility test was determined by Kirby-Bauer disc diffusion method, were assessed according to CLSI guidelines on Mueller Hinton Agar. Result: 65 Proteus isolates obtained from 4356 different clinical specimens, giving prevalence of 1.49%. Highest culture positivity was seen in pus samples (27.69%) followed by urine samples (24.61%). Antibiotic Susceptibility pattern shows highest susceptibility to Imipenem followed by Amikacin for other than urine samples. In case of uropathogenic Proteus, susceptible to Amikacin followed by Gentamicin. About 10.76% of the isolates were found to be ESBL producer and 9.23% of isolates were produced MDR. Conclusion: Proteus strain shows the maximum susceptibility to Imipenem followed by Amikacin and Meropenem. The high rate of ESBL in our study might be due to over use of beta lactum antibiotics. Low prevalence of MDR was observed due to fact that lesser number of Proteus isolate was obtained.
