MEDICAL MANAGEMENT IN PARIKARTIKA (FISSURE-IN-ANO)

ABSTRACT
Parikartika(Acute Fissure in Ano) is a longitudinal crack or ulcer in the lower end of the anal canal which is one of the painful surgical condition that effect majority of the population. Parikartika is not mentioned as separate disease but it is explained under the complication of Basti karma[1] and Virechana Vyapat[2] mentioned in classic. In present the era, due to changing lifestyle, such as sedentary work pattern, increased stress, improper dietary and sleep habits are the main cause. Apathyakara Ahara Vihara leads to Vitiation of Doshas takes place in Pakwashaya leads to Agnimandya than Vibandha(Constipation) that results in Hard stool passage which is painful and resulting in fissure- in- ano. Other clinical feature includes
burning, pain during and after defecation, which is lasting for half to one hour along with streaks of blood associated with spasm of the anal sphincters. Ayurvedic management of parikartika includes by using Madura, Kashaya, Snigdha and Sheeta Dravyas. In the form of Pichha basti /Sneha Basti. Tailpoorana, Pichhudharana are given prime importance in the management. Which basically relaxes the anal sphincters does Vatanulomana, Vedana shamaka with no any complication Sneha basti is the effective treatment with wonderful results.
