ABSTRACT
Diabetic plexopathy is well known clinical entity with lumbosacral radiculo-plexus involvement. But significant isolated brachial plexus involvement is extremely rare in diabetic mellitus in absence of any other cause. Diabetic plexopathy is usually asymmetric to start with, presenting with typical pain followed by atrophy and weakness of involved muscles over a period of months. We present you a 60 year male patient with isolated pain at left posterior aspect of shoulder region, bicep region with significant atrophy of tricep, infraspinatus, deltoid and other muscles surrounding shoulder joint and forearm with varying proportion with uncontrolled diabetes mellitus (HbA1c 15.7). This patient not had lumbosacral or right brachial plexus involvement.
So isolated brachial plexopathy can be caused by uncontrolled diabetes mellitus should be kept in mind.
