ABSTRACT
Necrotizing fasciitis in the perineum with extensive involvement of the peri-anal spaces is presented as painful generalized swelling, bulla formation with purulent discharge from multiple openings. The condition is characterised by a very quick progression to extensive sloughing with significant morbidity and mortality, affecting the subjects having the poor general condition. Early diagnosis is often difficult due to the similarity with conditions like peri-anal boils, abscess, and Ischio-rectal abscess. Patient is usually reported late with significant acuteness. Early diagnosis, extensive management are the main keys for the successful management and preventing the life threatening complications. A very effective wound management could be done by incorporating the basic fundamentals of Shashti-upkrama through the Ayurveda in such cases so as to have effective wound (desloughing) debridement along with rapid wound healing.
