Abstract

A CASE REPORT ON CEFPODOXIME- INDUCED HYPERSENSITIVITY REACTION (TOXIC EPIDERMAL NECROLYSIS)

ABSTRACT

Cefpodoxime is a a third generation cephalosporin antibiotic, which acts actively against gramnegative organisms. It is used in mild to moderate infections; because of its long half-life.its mechanism of action is inhibition of cell wall synthesis which reduces the cell wall stability and thus causing membrane lysis. The common side effects of cefpodoxime are diarrhea, swelling, redness, irritation, burning, hives, a return of fever, easy bruising/bleeding, signs of kidney problems and other hyper-sensitivity reactions and anaphylactic reactions. We report a rare case of cefpodoxime induced hypersensitivity (toxic epidermal necrolysis) reaction in a 23 year old female patient who came to hospital initially with complaints of drowsiness, face swelling and erythematic, B/L eye reddish color, total body reddish in color and burning sensation and skin pealing, where SCORTEN SCORE shown 1 and that resulted mortality rate as 3.2% initially after the SECOND day 3RD DOSE administration of MONOCEF-O 200MG-BD oral, which was prescribed to treat infection when patient is suffering with fever by a registered medical practitioner (RMP),on progress the poor prognosis was seen where the SCORTEN scale shown 2 score and 12.1% mortality rate and further resulted in death of the patient. Pharmacovigilance should be a part of patient care in order to identify and evaluate unreported adverse reactions which come out on usage of it by different people having different sensitivities which might not been seen during clinical trials. and also reduce occurrence of adverse drug reaction and encourage practitioners and other health care professionals in reporting adverse drug reactions, more over sensitivity test is most preferable option while prescribing certain antibiotics to the patients to know whether patient can tolerate or sensitive to the particular drug and depending on that, prescribing of antibiotics should be carried out to avoid adverse events.
