Abstract

CORRELATION BETWEEN FAST FINDING AND INTRA OPERATIVE FINDING IN MULTIPLE TRAUMA VICTIMS (BLUNT TRAUMA) IN SULAYMANIA EMERGENCY HOSPITAL

ABSTRACT

Background: Focused Abdominal Sonography for Trauma (FAST) is widely applied in the initial management of trauma patients. Being simple, non-invasive and without risk of irradiation, make it attractive tool in evaluation of trauma patients. Objective: Evaluated FAST sensitivity, specificity and accuracy in detection of intraabdominal fluid and organic injury in blunt abdominal trauma victims. Methods: Prospective study conducted in the Emergency Hospital of Sulaymania in Kurdistan of Iraq from 1st of January 2015 to 1st of July 2015 period. The FAST done by senior radiologist or postgraduate student during the initial management of blunt abdominal trauma victims with equivocal clinical findings. Results: One hundred nineteen 119(100%) patients included in the study, with over all do FAST exam in the emergency hospital about this 91(76.5%) patients has normal FAST finding but 28(23.5%) patients has abnormal FAST findings. Among these abnormal finding 11(9.2%) patients underwent operations and then determine the sensitivity, specificity and accuracy of FAST exam in the detection of intraperitoneal fluid in multiple trauma victim patients as (52.94%), (81.37%), (77.31%) respectively with significant P-value 0.002. Finally the senior’s judgment of FAST exam is a highly sensitive, specific and more accurate this result is significant with P-value less than 0.001. Conclusion: FAST ultrasound is very helpful for assessment of blunt abdominal trauma and to detect intraabdominal fluid in emergency departments. FAST are moderately sensitive but specific and accurate in detection of intraabdominal fluid after blunt abdominal trauma patients, these scans are highly sensitive, specific and more accurate when performed by seniors.
