Abstract

ASSESSMENT OF SENSITIVITY PATTERN OF CEFEPIME ALONE AND CEFEPIME/ SULBACTAM IN COMBINATION, AGAINST ESBL PRODUCING GRAM NEGATIVE ISOLATES FROM A TERTIARY CARE HOSPITAL. A STUDY FROM AHMEDNAGAR, MAHARASHTRA.

ABSTRACT

The Beta-lactam and Beta- lactamase inhibitor in combination is currently the most successful strategy for circumventing the resistance mechanisms like production of ESBL by gram negative isolates of clinical specimens. The study therefore aims to assess the sensitivity pattern of cefepime alone and cefepime/sulbactam combination against ESBL producing gram negative isolates from a tertiary care hospital. Total 186 specimens were selected randomly from which 66 gram negative bacilli were isolated. Out of 66 gram negative isolates, 38 were detected as ESBL producers. Out of 38 ESBL producing isolates, 07 were Pseudomonas aeruginosa, 02 were Acinetobacter spp., 14 were E.coli, 11 were Klebsiella pneumoniae, 02 were Citrobacter freundii, and 01 isolate of Proteus mirabilis and Proteus vulgaris each. Out of 07 isolates of Pseudomonas aeruginosa 01 isolate showed resistance to cefepime and cefepime/sulbactam combination and 4 isolates were resistant to cefepime. The isolates of Acinetobacter spp. showed resistance to Cefepime and 01 showed resistance to cefepime/sulbactam combination. Out of 14 isolates of E.coli, 13 were resistant to cefepime while 3 showed resistance to both. In 11 isolates of Klebsiella pneumoniae 9 were resistant to cefepime and 4 showed resistances to cefepime/sulbactam. The 2 isolates of Citrobacter freundii showed resistant to cefepime and one was resistant to cefepime/sulbactam combination. Proteus mirabilis and Proteus vulgaris both were sensitive to both the antibiotics. Study suggests, combination of beta-lactamase inhibitor and beta-lactam antibiotics should be encouraged for further in-vivo studies as that can make some good therapeutic aid.
